Documents Required from Newly Enrolled Continuing Education Division Students and Related Notices for the 115th Academic Year, Ming Hsin University of Science and Technology

I. Materials to be submitted to and processed by the Continuing Education Student Affairs Section:

1. Student Basic Information Form (Attachment 1) — please affix a photo and complete both sides of the form.
2. Military Service Survey Form (for male students only, Attachment 2).
3. Students intending to apply for student loans or tuition and fee reductions/exemptions, please refer to the "Ministry of Education Student Financial Assistance Measures" section on the Continuing Education Student Affairs Section website:

II. Materials to be submitted to and processed by the Continuing Education Academic Affairs Section:

1. A copy of educational credentials or the original certificate of completed coursework.
2. Two 2-inch color photographs (please write your department/program and name on the back of each).
