Student Leave Application Form
Leave Application Regulations: Students who are unable to attend classes for valid reasons must complete the leave application procedures in accordance with the following regulations.
I. Online Leave Application: Online applications must be completed within two weeks. Only personal leave and sick leave are accepted through the online system; for all other types of leave, please submit a paper leave form. Please access the "Continuing Education Division Student Online Leave Application System" via the link at the bottom of the university's official website. Do not search for the system directly on Google.
II. Paper Leave Application: Paper applications must be submitted within two weeks. Please print the leave form from the website yourself and deposit it into the "Leave Application" mailbox located in the central corridor on the 1st floor of the Ming Ming Building. After review and processing by the Continuing Education Student Affairs Section (approximately 1–2 business days), the form will be returned to the class mailbox, and students should collect it from their class representative.

IV. Leave Applications for Midterm and Final Examinations: Please go directly to the Continuing Education Academic Affairs Section to complete an [Examination Leave Form].
